By October 2014, all area emergency rooms will refer patients who do not have an emergency condition to the Grayson County Primary Care Clinic between 9 AM and 8 PM every day.

Ensure that non-emergency patients have transportation to the Clinic through the Texoma Area Paratransit System (TAPS)

By October 2014 Clinic hours will be daily including Saturday and Sunday between 9 AM and 8 PM, with enough healthcare personnel to address all patients' needs.

TRANSFORMATION PROJECT GOALS

Community information campaigns will be held to ensure areas residents know how to contact and utilize the Clinic and avoid the high cost of an emergency room visit.

Patients will receive both healthcare and self-care information to improve their health status in areas including heart disease, obesity, diabetes and health risks linked to those conditions.

This Clinic will become the medical home base for its patients.

The Grayson County Health Clinic is a place where local hospitals work together to provide primary care to adults and children without insurance or with low incomes. These are Texoma Medical Center in Denison, Texas and Texas Presbyterian Health-WNJ in Sherman.  We also work with Grayson County government, the Texoma Health Foundation and the Wilson N. Jones Foundation. Our services are coordinated with the Grayson County Health Department and other medical and healthcare organizations in Grayson County.

There may be a sliding fee scale depending on your income. We are the medical home for the Grayson County residents covered under the County Indigent Health Program.  We accept Traditional Medicare, Texas Medicaid and CHIPS.

Clinic hours are Monday-Friday 8 AM to 12 noon, and 1 PM to 5 PM. 

A Saturday clinic from 8am-12pm for walk in appointments will open soon.
